Primary Healthcare Centers is seeking a Family Nurse Practitioner (FNP) to join our team at our Gilbert Elementary clinic location. This position provides primary care services to pediatric and adult patients in a collaborative, community-focused healthcare environment.

Our mission is revealing God’s love through high-quality and accessible care for all, and we are looking for providers who are passionate about serving others and making a difference in their community.

Responsibilities
  •  Perform physical exams and preventative care visits 
  •  Diagnose and treat acute and chronic illnesses 
  •  Order and interpret diagnostic tests 
  •  Develop and manage patient care plans 
  •  Prescribe medications and treatments as appropriate 
  •  Refer patients to specialists when needed 
  •  Maintain accurate medical documentation 
  •  Collaborate with physicians and healthcare staff to provide quality patient care 
Qualifications
  •  Current Family Nurse Practitioner (FNP) license
  •  Board Certified Family Nurse Practitioner 
  •  Active DEA license or ability to obtain 
  •  Strong clinical and patient communication skills
