Primary Healthcare Centers is seeking a compassionate and dedicated Family Nurse Practitioner (FNP) to join our team at our Trenton clinic location. This position offers the opportunity to provide high-quality primary care to patients of all ages in a collaborative, mission-driven environment focused on improving the health of the communities we serve.

What You’ll Do
  •  Provide comprehensive primary care to pediatric and adult patients 
  •  Perform physical exams, wellness visits, and preventative care 
  •  Diagnose and treat acute and chronic conditions 
  •  Order and interpret laboratory and diagnostic tests 
  •  Develop and manage patient-centered care plans 
  •  Prescribe medications and treatments as appropriate 
  •  Refer patients to specialists when necessary 
  •  Maintain accurate and timely medical documentation in the EMR 
  •  Collaborate with physicians and healthcare staff to ensure high-quality patient care 
Qualifications
  •  Current Georgia Family Nurse Practitioner (FNP) license
  • Board Certified Family Nurse Practitioner
  • Active DEA license or ability to obtain 
  •  Strong clinical judgment and patient communication skills 
  •  Passion for community-based healthcare

Compensation: $100,000.00 - $110,000.00 per year
