Primary Healthcare Centers is seeking a compassionate, organized, and patient-focused Referral Navigator to help patients access specialty care and services outside of our clinics. This role is ideal for someone who enjoys helping others, coordinating care, and building relationships with patients and healthcare providers.
What You'll Do
- Coordinate specialty appointments and diagnostic testing for patients.
- Assist patients with scheduling, transportation, and financial resources.
- Communicate with specialty offices and transmit referral documentation.
- Track referrals and appointments within the electronic medical record system.
- Follow up with patients and providers to ensure continuity of care.
- Collaborate with providers, case managers, and care teams to support patient needs.
Qualifications
Required:
- Knowledge of Microsoft Word and Excel.
- Basic understanding of medical terminology.
- Strong communication and organizational skills.
- Ability to maintain patient confidentiality.
Preferred:
- Referral coordination experience.
- Medical office experience.
- Experience with electronic medical records (EMR).

Compensation: $17.00 - $18.50 per hour
